It is helpful for our office to receive your patient information prior to your appointment. Patients will generally receive a text link to complete electronic forms prior to their appointment. You also have the option of downloading forms from our website and completing them manually, which requires a PDF reader (like Adobe Acrobat). You may email completed forms to scheduling@entlubbock.com, fax them to 806.788.0470 or simply bring the completed forms to your appointment. If you prefer to fill them out in our office, we recommend you arrive 15-20 minutes early for your appointment.

Note that there will be a $35 fee for appointments missed or cancelled with less than 24 hours notice. Our office closes at noon on Friday, so Monday afternoon appointments need to be confirmed before then to avoid a cancellation fee. Please be considerate when canceling or rescheduling an appointment so that we may have enough time to accommodate other patients on our wait list. Thank you for your understanding.
